In the example URL "surveys.ezfastforms.com" the Subdomain section is the word SURVEYS. Your Subdomain might be the acronym for your company name, like we use EZFF or EZ. Your Subdomain selection must be uniue in our system, and you can Check The Subdomain to make sure you can register it with us. Subdomains must be 1 - 25 characters in length.
